Don’t send original documents – send either photo copy of notary public’s sig/stamp and/or vital records/county clerk’s sig/stamp or a list of notary public/vital records/county clerk’s names and commission numbers (listed on stamp). You also need to tell them the date of the documents were notarized. They will send you the state certification forms to attach to the originals.

NOTE: Iowa prefers you get all Vital Records from the Main Office in Des Moines, NOT at the County Clerk’s office. This way, they can walk your record over to the Secretary of State’s office for the State Seal

Note: The Des Moines Field Office is closed to the public on Wednesdays.
